Assessments are conducted in person in Toronto, Ontario
(2069 Danforth Ave, Suite 2).
Please complete all sections of this form so we can add you to the correct waitlist.
For children in French programs, Dr. Wallis will be happy to provide a brief phone call to answer questions and determine next steps. Please note that the wait for a phone call is 2-4 weeks. Thank you for your patience.
For children in English programs and interested in working with Dr. Sagar, we will be happy to provide a brief phone call within a week or two of completing this form.
